CATALYST NUTRITION AND TRAINING

One of our primary reasons for starting Catalyst Nutrition and Training was to be able to accept insurance and make nutrition counseling more accessible to everyone.

 

Below, you will find lots of helpful information on using your insurance to see us. We are also happy to accept out-of-pocket payments for those without insurance or whose insurance doesn't cover nutrition services.

  • UPMC Health Plan

  • Aetna

  • Blue Cross Blue Shield

  • Highmark

Insurance We Do Not Accept:

  • Medicaid

  • Geisinger Health Plan

How to Know if Your Plan Covers Nutrition Services

Every insurance company is different when it comes to nutrition coverage. It's critical to find out if your specific plan includes coverage. We are happy to help, but here are some instructions to get you started:

  • Prior to your appointment, please contact your insurance company to ensure you have met your deductible and that nutritional counseling/medical nutrition therapy services are covered.

​

  • If asked for procedure codes to confirm coverage, you can provide the following:

    • ​97802

    • ​97803

    • 99404

 

  • The insurance company will be able to tell you if you have coverage, what your copay is, if you have met your deductible, and how many visits you receive in a calendar year.

What to Check Before Your Nutrition Visit with Us

You must check insurance coverage before coming in for your appointment. Even if you have an excellent insurance policy, it does not mean you have coverage for nutritional services.

 

To verify your nutrition coverage, please call the 800 number on the back of your card and ask to speak with a representative from member services. The provider for the visit is Brooke Evans MS, RDN, LDN.

Please ask the representative the following questions:

​

  1. Do I have nutrition services on my current insurance policy?

  2. Is my diagnosis covered? [You can get this diagnosis from your doctor]

  3. How many covered visits do I have per calendar year?

  4. What is considered a calendar year for my current policy?

  5. Do I have a co-pay or deductible for nutrition services?

​

You will be asked to pay any applicable co-pays and/or deductibles at the time of your visit. You will also sign a liability waiver stating that you will be responsible for the charges if your insurance does not cover the visit.

​

*note: we will collect a credit card number at your first visit. In the event your claim gets denied by your insurance company (or if you have an outstanding co-pay/deductible), your credit card will be charged for the visit. Therefore, please make sure to verify your insurance PRIOR to your visit. We assume no responsibility for the verification of your benefits.

Insurance FAQs

What happens if you participate with my provider – but when I called the insurance company they told me I have a yearly deductible I have not met?

​

In the event you have a deductible that you have not met, we will send you a bill for the cost of services. This will allow you to “pay down” your deductible. Once your deductible has been met and you have nutrition services on your policy, we can then directly bill your insurance company.

 

What happens if I have insurance that you do not participate with?

​

In the event you carry insurance for which we are considered out of network, you will need to pay for your counseling session at the time of service. We are happy to provide you with a “superbill” and/or an itemized invoice you can submit to your insurance company for reimbursement. Reimbursement is based on your policy’s standards. Please call your insurance company’s member services number to verify how reimbursement works under your current insurance policy.

What forms of payment do you accept?

​

We accept cash, check, or charge. Please make checks payable to Catalyst Nutrition and Training.

 

Is there anything I should do to prepare for my nutrition visit?

​

  1. ​Verify your insurance benefits with your insurance company

  2. Fill out and sign all forms on the Healthie platform. We recommend downloading the app for easy communication in the future!

  3. Be prepared to provide a credit card that will be kept on file on our secure, HIPAA-compliant platform. Your credit will only be charged in the event there is a problem processing your claim.

  4. Please call us with any questions at (888) 862-0803.

Other Insurance Notes

  • You are responsible for updating Catalyst Nutrition and Training of any changes or updates to your insurance policy. Updated insurance information is necessary to have your appointment covered by insurance. Failure to do so will result in insurance claims being denied, and you will be personally responsible to cover those visits.

  • It is critical that, if your insurance policy changes, you re-verify your nutrition benefits for Medical Nutrition Therapy on the new policy as your benefits may have changed. Failure to verify MNT benefits may result in insurance claims being denied, and you will be personally responsible for covering those visits.

​

  • You assume responsibility for knowing how your individual insurance policy works and for presenting any necessary referrals and other documents to Catalyst Nutrition and Training.

  • Copayments must be paid at time of service.

​

  • Some insurances require a referral from your physician to bill for Medical Nutrition Therapy. This must be presented to Catalyst Nutrition and Training during the time of service in order to be covered by insurance. You can download a referral form below to provide to your doctor:

  • If your insurance policy is not in effect, required documents are not provided, or Catalyst Nutrition and Training does not participate with your insurance, payment is expected in full at the time of the visit.
